Community Resources

***The Outer Cape Health Services Community Navigator program helps assist residents with unmet medical, behavioral health, or human service needs through collaboration with local agencies, providers, and community groups. The Navigator works closely with community agencies from referrals and service coordination to help clients in their community by building relationships and identifying support networks.

The Outer Cape Health Services Community Navigator program can:

–Refer and connect to community resources such as social services, mental health services, substance abuse services, homelessness/ housing assistance, medical care, and case management.

–Problem-solve barriers to access

–Assist in navigating the variety of services available

–Coordinate care

The Community Resource Navigator program accepts residents of all Lower and Outer Cape towns regardless of insurance. Referrals for the CRN program can be made by residents themselves, family members, service providers, or community members.

***The Wellfleet Food Pantry has been serving our community since 1999. In 2016 the pantry provided food to more than 125 families (over 300 individuals), helping them through lean times and freeing a little money for other needs. These numbers represent more than 10% of Wellfleet’s year-round population.

The Wellfleet Food Pantry is located in the Grace Chapel at the corner of Route 6 and Lieutenant Island Road in South Wellfleet. Hours are Monday 4:30-6:30, Tuesday and Thursday 10:00-noon. First time users must register with the Lower Cape Outreach Council by calling 508-240- 0694.
